iPad Safari check boxes, quiz buttons and quiz answers disappearing

I am testing on an iPad 3 with iOS 9.3.2. The course runs beautifully in HTML5 on every other platform tested. iPad, Safari in particular, is having a lot of issues.

1) Variably, check boxes in quizzes will either not appear (still clickable even though not displaying visually), appear on click, or disappear on click. This also happens in Chrome

2) Sometimes the Submit/Previous buttons in quizzes do not appear. I have tried using both Transparent and Image button types

3) Random multiple choice answers will not be present. Sometimes more than 1 missing on a slide

4) Quiz scoring is out of whack (posted another topic about this here: iPad Quiz Scoring Off )

I have searched and seen very little on this topic. Any type of advice or direction to potential solutions would be greatly appreciated!
