Is this a bug? Two issues with the Hotspot Question

In Adobe Captivate 9.0.1.320, I am having a trouble with the hotspot question.

When I set it to 3 attempts, when previewing, it seems I can only make two attempts. After this I cannot take another go, or click the submit or clear buttons anymore.

When this didn't work, I set it to infinite, whereupon I discovered the second issue:

When I preview the questions in the browser (Chrome), I am not able to use the submit button more than once unless I also allow a 'retry' button to appear. It seems to me that it should be possible to set a question to infinite answers without having to have a 'retry' button appear each time.

Can you help with either of these issues?

Thanks

Have you removed any default text boxes or components from this quiz question?  E.g. the Question Title or Question Text captions?  If so, that could be the reason. Delete the current question and rebuild a new one.

The only thing I changed was that I made a couple of boxes transparent on the master slide. I didn't remove any boxes though. Not sure what that little blue icon with lines in brackets means though. Is that a clue to the problem?

That icon is just the indicator of a placeholder object, no worries.

Is this a CPTX or CPTL file?

CPTX. Weirdly it now seems to have started working again when I set it to 3 tries. I am assuming that the recent update fixed something but for some reason it didn't work until the program had been reloaded a couple of times.

Thanks for your help.
