CSV processing failed. Please upload again. ERROR_OTHER

Dear Community, 

I've added some columns/fields to our CSV file that we use to upload users to Captivate Prime, and now the file will not upload.

 

If I remove the new columns, it uploads successfully.

 

Any ideas on why I cannot add columns to the csv?
