head and sholder wierd attachment problem

in the video as you see head and the left part of shoulder has been attached but the right side not!!!

I couldn't understand the reason?

Just following up here (we solved this on Twitter) - by reducing head tilt and position strength we were able to get it working better. But because this character essentially has no neck, drawing in more artwork for the shirt/neck area behind him would help with more extreme bends/tilts.
