Issue with puppet shoulder when moving arm

Hello,

I am a novice at this so please bear with me.

I commissioned a puppet to be designed.  After I walk through the oksamurai training on rigging I am getting the following issue:

Any idea of what I am doing wrong?

Have you tagged the arm with shoulder, elbow, and wrist yet? (Select the little circle and look at the stick figure to the right and select the correct tag?)
