failure at step 3 setup. code 2134.

  Flash player work on all sites but one. Worked there for years before but suddenly I get this code. I have done all suggestions so far. What is code 2134?

That's typically about storage. Try this:

1. Go to the Flash Settings menu Adobe - Flash Player : Settings Manager - Global Storage Settings Panel
2. Set the slider up to any amount above 0 (KB)
3. Check the Allow third-party Flash content to store data on your computer checkbox.

This issue usually happens when sharedObjects get cached just like any other part of a site can be cached and the object is invalid or unable to write itself to your browser. You can always try erasing your browsers site cache if the object is invalid that it's loading.
