The cfdocument tag in  Coldfusion 8,0,1 does not support <b>. I got an error message from Adobe Reader X plugin "There was an error opening this document.The file is damaged and could not be repaired". When I replaced <b>with <p style="font-style:bold;"> my html rendered to a pdf.

Your first example, "Using an image file URL", does not work, because your tag names should be "img", not "image".
