The image is untagged, so it assumes the RGB Workspace which is sRGB. 

The Safe for Web dialog shows you converting to sRGB (which does nothing as the image is sRGB) but you preview with Monitor Color. 

 

Did you previously work in Adobe RGB possibly? 

What are the thre color fields’ values?

Maybe, Idk... I mean, when I have noticed this issue I have tested a couple of different color profiles but they made colors look washed out or oversaturated. As for these bars from the example I have used:

Red- #da4963

Yellow- #ffd739

Blue- #0064ff
