Unable to combine 10 medical files--just end up with 10 copies of 1 file

I have been able to combine a medical file up until recently, comprised of 10 pdf document notes and approximately 8 photos into a "binder" file.  Now it creates a binder with 1 note copied 10 times along with all the individual photos.  As far as I know nothing changed except possibly since the computer updated, but I'm not sure if it predated the update.  I have tried several approaches using Tools->combine while in Acrobat and then selecting files, or "select all" in the documents then right click and "combine files in Acrobat," and then no matter what I do I watch as the program seemingly puts all files into the binder (watch as it checks off each note, and then I double check the pictured documents to make sure they are different notes).  Then I click on "combine," and I always get 10 copies of one document instead of the 10 different documents.
