Creating new Integration - Console issue?

I am trying to create an integration to the View SDK, but I cannot find where to specify the application domain.

 

Also, I have an integration that was created 2-3 weeks ago, that displays the domain, but subdomains are not able to access.

 

What am I doing wrong?

I ended up with a configuration issue causing the subdomains to fail.  That has been resolved, but I still can't see how to create a new integration and specify the application domain.

View SDK integration can’t be created via Console. “Click here to access” link (shown with View SDK service option on Console) takes you to the View SDK home page, where you can create View SDK Credentials and also go through the documentation.
