Who actually decides which new functions should be implemented and how the design of the new interface should look like?

For example, for the new Creative Cloude Desktop App .....

Who makes decisions?  The usual suspects.  Upper management and that  guy/gal with the really nice office on the top floor.

I would love to say: that's user driven. But that's ony half of the story. There is also a lot of programers ego, managers ego and market ego in. Some features are more interesting than others, easyer to implement and get new customers. So yes, it's probably all of those.
