I am sending out a document requesting signatures, and I need the person receiving this document to select one of two checkboxes only. When add two "set as checkbox" boxes, it forces the person signing the document to click both... How do I make this form to only allow one check box and not both?

Which Adobe app or service are you using to create the form document?

Why don't you use radio buttons instead of checkboxes? Radio buttons are better for either/or type questions.
