Need help reactivating CS4

I had to deactivate my CS4 to install a new hard drive on my computer so I went to my "about  my Adobe" and copy and pasted all the information on it and saved it so that I could reinstall it and reactivate it. Now that I tried to do that turns out, the serial number it gave me / saved is only 16 numbers and it requires 24. I no longer have the cds for my program and do not have the full serial number. Below is attached information that I saved.... Is there a way to reactivate my account based on the information I have?? I can send the information, just let me know!
