Dreamweaver Contact Form ... Help with PHP or whatever code is needed to get the "submit" button to send e-mail (please)

I have been trying all day to get a contact form in a webpage I'm doing to send to an e-mail address. I've done various tutorials and tried many ways but can't get it to work. I'm attaching the dreamweaver contact htm file and the PHP I was trying to get to send to an e-mail.

Basically I know that IN the code in the DW contact htm it has to point to the filename of the PHP. Then the PHP has a place to put your e-mail address you want to send to. And all the form txt areas have to be named correctly in the PHP and htm ... but I just can't get it to work properly.

Could anyone please help? Thanks,

For starters, you need to include more details. What are the symptoms? Is there any error message? Is the user redirected to a thankyou page? It's also helpful if you provide the url to the form, and include the php script.

Thank You, you have no idea how greatful I am. And how stupid I feel.

You see I went to upload the contact html and php on the site to show you it wasn't working. I didn't realize it doesn't work when just viewing in a browswer from DW ... but once I uploaded the file to my host/site it works!!! OMG I feel stupid, but thanks for making me figure that out. appreciated.
