Dynamic Checkbox - driving me mad

I have a checkbox on my admin page. I want it to display the status of the
record field in the database, either a 1 or a 0 (if 0 then checked if 1 then
unchecked)
I also want to update the database field with the same checkbox.
Anyone know the code for this??
