Form2Mail email

I'm using HotDreamwaver Form2Mail and am having a problem with the email received from the form. The email is replying to the email address specified in the script instead of the email address of the person filling out the form. I've followed the instructions in their FAQ #194 and have managed to change the reply address so it displays the user's email address, but if I reply to the message received, it goes to the email address specified in the script.

Eg:

- email address in script is webmaster@domain.com

- reply email address is person@anotherdomain.com

- I receive the email from person@anotherdomain.com, hit reply, and the email goes to webmaster@domain.com instead of person@anotherdomain.com

Can someone please tell me how you get the subject line to auto populate in Adobe 11 we were on 8.1 profession when you saved the document as a PDF the named report would also appear as a subject header within outlook this is now not happening in the lasted versions.
