Variation Selection in PHP

I am working on a WordPress site and would like to implement the variation selection functionality from this link Karton Çanta - Süperform Matbaacılık in PHP. I would appreciate your assistance with this.

I need to add the selected variation to the cart in a way that generates an order form.

This is not a Dreamweaver issue.

Using a WordPress site means using WordPress to make functional modifications.

As @BenPleysier said, this is not a Dreamweaver question.

You should start with your Cart Provider.  Ask them if your shopping cart can support this functionality and if so, how.
