I have a dita map, which I have created manually. It creates PDFs and HTML with or without FrameMaker and as expected it has the look of dita-ot pdf2. In FrameMaker: Structure->DITA->Generate DITA-OT Output...
However, I would like to use the dita bookmap option of FrameMaker in order to include the template files, such as the cover page, table of contents etc., into my book. When I use the dita map together with the publish option in FrameMaker, two issues come up: 1. after FrameMaker publishes the pdf, it crashes/hanging and 2: the published pdf creates page breaks after each of my topics and the numbering is not fine. This is how my dita map is made:

Problem solved: I had to select Generate flat Book hierarchy under DITA Options.
