Hi,
I have run into this problem of late.
The TOC links in FM 11 work fine (Ctrl-Alt) but when converted to pdf the links are not working at all.
I use Adobe PDF reader XI for generating the pdf.
Please ler me know where I could be going wrong.

Are you printing to the Adobe PDF printer instance or doing Save As PDF? Have you got the full Acrobat installed or just the standalone PDF creator that ships with FM?

Are there any character formatting changes in your TOC entries? Hold down the ctrl+alt and mouse over your TOC entries in FM. Does the pointing hand disappear at any point in each entry?
If the active hypertext zone in the FM file is confined to the first part of an autonumber sequence, then this may not propagate through to a link in the PDF, as FM sets the hypertext boundary at any formatting change.

I checked each entry in the TOC. The pointing hand icon symbol can be seen with each entry except for Chapter level entries
TOC looks like this:

The pointing hand appears at each level in the TOC but for ChapterNumTOC and ChapterNum_next_TOC and all the subsequent chapters in the TOC.
Added to this, the cross references in the book are not working as well.

Turn on your View > Text Symbols and check that there are markers (T-shaped symbols) in your ChapterNum TOC entries and also in your cross-references.

It sounds like some of these entries may have been manually added (or inadvertently deleted). FM would automatically insert the hypertext markers when it created the TOC.
I would use SaveAs to make a copy of the current TOC with a different name. Then I would regenerate the TOC from the FM book file. Also, check the TOC setup before that to ensure that all of the paratags required are present in the "Include Paragraphs Tagged" column.
With the cross-refs, if they are missing markers, you may need to manually re-link these as well.

Today's issues:
Clicking the Chapter 1 TOC entries lead to Chapter 2.
Some of the cross references still do not resolve.
I have followed all your suggestions but these seem to persist.

Ignoring the chapter numbers, are the topics that you're jumping to the correct ones? If so, then it's an numbering issue with the chapters. Set all numbering controls at the book level (as these always override individual chapter settings) and try to use the <$chapnum> building block wherever you need to refer to a chapter number. Also check your TOC reference page to see if the correct building block is being used.
You can unresolved cross-reefs if a file doesn't open properly (missing fonts or other error or warning messages). Open all files in a book (use the Shift+File > Open all files... shortcut for the book) and then update your book. If there are still unresolved cross-refs, then these are probably broken (i.e. someone deleted the marker) and you should use the FInd > Unresolved Cross-References to locate these and manually re-link to the correct locations.

I have a similar issue on my files. When the PDF generates, the PDF bookmarks work just fine, but the TOC links do not. While the links are recognized in Framemaker (the "T"), I could not actually generate a PDF where the links would actually work. My solution was to rename the file (using the book view in Framemaker), to a shorter name. I realize it may not be the same issue, but posting this reply in case it helps someone.
I am using Framemaker 2015.
