Links in TOC and Index not working, Mini TOC does...

I'm making PDFs with hyperlinks for the TOC and Index files. The PDFs do not seem to have working links at first. I found another thread where a series of commands/characters were added to the reference pages, and I thought that worked but it didn't. 

In the mini TOC, the links all work fine. In the main TOC, I got links to "work" but they try to open up a completely different document instead of the same document that they are in. In the index, there are clickable links, but ONLY in the small spaces just before the page numbers, and they also want to go to a different document. What's going on and how can I fix it?

I am not sure about the going to a different document part, but the index makes sense because you have a character property change for the page numbers. The link will work up to the first character property change. Can you post a screenshot of your IX reference page?

If you have time to meet tomorrow, I can take a look at the TOC links.
