3. I suspect that the generated reference page does not have a Running H/F 1 or 2 in the associated Master Page. The LOT, LOF and AML all have the correct formatting after customizing the reference page. No idea where to include a frame to infuse the Running H/F variables. I made a few unsuccessful attempts, but don't want to disturb the documents structural integrity. This is probably rudimentry query for the crowd here.

The Running H/F variable is usually in a text frame on the master page which should be the header. It will not change the structure of a file.

My Master Page has a Running H/F 1 & 2 and works well in the main document, which is based upon the Report template.
I was just re-thinking the ask, given your input. If I separate the Title page into a separate document, then, I could position the LOT and LOF as front matter, where the H/F is less concerning, I suppose and I probably do not need to be as concerned. Initially, I wanted to include these documents at the end as part of the Appendix, as for the audience, that might work better.
The Glossary (AML) should remain in the Appendix as the last document. Regardless, I would prefer the H2 value "Appendix <n>" in the header and the TitlePage style in the Footer, to be consistent across the document.

Running H/F variables go in master page background text frames. Background text frames don't have a Flow tag.

Thank you @frameexpert. My Master Pages has a Running H/F on both the Left and Right pages. When creating the generated documents (LOT, LOF, AML), they lack this data. I'll look again at the Flow tag. This document is based upon the Report template, which I like.

I use the AML for a Glossary. Works quite well. If you use a standard LOM, the Glossary list is ordered in ascending order basedupon appearance in the book. Fine for a small list. After a full page, AML is preferred for my client.
