TOC Autonumbering Issue - Not Picking Up Chapter Numbers

Hey gang,

I'm building a TOC across a range of FM docs and it's not picking up the chapter numbers.  I want the TOC to look like this:

1 Introduction.....1-1

1.1 Description...1-2

1.2.1  Interface...1-3

Instead, what I get is this:

Introduction 1

Description 2

Interface 3

The reference page is arranged liked this:

<$chapnum>\t<paratext>\t<$chapnum>-<$pagenum>, with a tag of Heading1TOC

<$paranum>\t<paratext>\t<$chapnum>-<$pagenum>, with a tag of Heading2TOC, Heading3TOC, etc.

The headings in the body are arranged like this: H:<$chapnum>.<n>.<n>...<n> Autonumbering is correct throughout the chapter files; it just disappears in the TOC.  In addition, the TOC isn't picking up the tabs from the reference page whether it has a manual tab or the \t shortcut.  All of the tabs are correctly set in Paragraph Designer.

TL;DR: the TOC isn't autonumbering correctly and it isn't picking up the tabs from the reference page.  Any ideas?
