Using Publish to make MS Help files; one chapter crashes it

I am using the Publish function to turn a book into an MS Help file (.chm).

When it hits one chapter, I get a "Failed Importing Documents" message and it stops. If I exclude that chapter, it publishes the rest of the book.

Clearly I need to look for something that makes that chapter different from all the others. Where should I start looking?

I've already copied and pasted the same format to all chapters.

I've already copied and pasted the same format to all chapters.

Have you tried the opposite approach? Open one of the working chapters, rename it, and replace the content with the content from the non-working chapter? Does that make a difference?
