Volume number and formatting issues in TOC

The screen capture below shows the Word TOC I'm trying to replicate in FrameMaker.

The volume number and three column headings will not display in my FrameMaker TOC. I tried including both on the reference page (see below).  My chapter info is coming from the reference page, so I know I have that set up correctly.
