FM 2020 - DITA 1.3 - ToC does not present correct chapter numbering - does not match the book

Greetings,

My ToC does not reflect my book. The titles are correctly ordered and appear at the appropriate level.

The layout is correct. I removed the word Chapter from the title.0 paragraph format and I added a numbering scheme to the title.1 paragraph format (to match the model used in my document).

I edited ditafm-output.ini to look like this:

I can get the layout I'm looking for, but I have two problems.

First, my title.0 entries go from 1, 2, 3 on to 2, 2, 2 for the rest of the book.

Second, my title.1 entries all use 1 for the chapter number.

So I get something that looks like this:

Any assistance is appreciated.
