Save Ditamap as Book with FM Components: Reference page graphic frames in ditabase.template.fm do not import into the created .fm file

I have a structured DITA 1.2 application. I created DTDs, EDDs, Rules, and template files for concept, task, reference, topic, and ditabase.

These files are located in C:\Program Files (x86)\Adobe\AdobeFrameMaker12\Structure\xml\DITA_1.2\app\technicalContent.

I use the Save Ditamap as Book with FM Components option to get the book with TOC, frame file(s), Index, etc.

The ditabase.template.fm used by the save ditamap process has graphic frames on the reference page for hazard message icons.

This file resides in the location noted above and in C:\Program Files (x86)\Adobe\AdobeFrameMaker12\Structure\xml\DITA\outputTemplates.

After saving the ditamap as a book file with FM components, I do not see the graphic frames on the reference page in the frame file that is created via the save as process.

Is there a setting in the ditafm-output.ini file (C:\Users\<name>\AppData\Roaming\Adobe\FrameMaker\12) that will allow them to come through?

The original TemplateImportFlags was set to PGF|FONT|PAGE|TABLE|COND|REFPAGE|VAR|XREF|COLOR.

I changed the the setting to All and that did not work.

Is there a rules or other processing file that gets used by the Save Ditamap as Book with FM Components method?

Any ideas?
