Is there a way to make an asset in Illustrator that can act like a prepared form in Acrobat?

When I make drawings for client in Illustrator, I have a line on the drawings where my clients are supposed to sign when they approve the drawings. 

 

In Acrobat, there is a function to "prepare a form" where you can choose certain form fields that are automatically ready to be filled. Is there a way I can set something up like that in Illustrator so that I don't need to save my Illustrator file to pdf, open it in Acrobat, and then create that form field?

You cannot make a form in Illustrator.

But you can make the form yourself in Acrobat so your clients don't have to.

You can prepare parts of your design in Illustrator, use and combine them in InDesign to prepare a PDF form:
