It seems to have happened mid-session of me working in Illustrator, but what was once the easy tool to get outlined shapes has now become a problem. It minimized what it "outlined" which was good or bad depending, but now it has added shapes that aren't even there (on a simple black and white original). Did I select something I didn't know about for this to happen, or is this an update? It needs to go and has now just made my life much less efficient. Picture attached.

And another, Attached you can see the original I am trying to grab the black part from, and then after Object > Image Trace > Create and Expand, how much extra it added. It used to literally just take the flat image and outline the black and white as solid parts. Please help.

There are problems with the current Image Trace as you found out.
You can go back to a previous version like 27.5
In the CC app go to Apps > All Apps
Next to Illustrator you will see 3 dots
Click and select Other Versions
You can Uninstall Illustrator and install the previous version or leave the current Illustrator installed and install an additional previous version.

Try to go back unto one that works and reset the preferences.
