How to curve a square to match a candle label

Hi All!

I'm a novice Illustrator user and I'm trying to recreate a label on a candle but I can't seem to figure out the best way to do this.  The original resolution of the label was bad so I'm trying to create a new label on the photo.

I've attached the file, any advice would be greatly appreciated - it's for a school project.

You can use 3D, and map it.
