My illustrator appears like this at start-up:

I'm not sure if I pressed a shortcut key to trigger this. Please help me revert to original interface.

You can turn the Home screen off in the Prefereences.

Do you mean the strange text fields?

I've not seen anything like that before. Have you tried restarting or resetting preferences?
