All of a sudden, I can't print 6 x 12" documents from InDesign. The print dialog box diagram shows my custom size document will be printed on letter size paper. If I change the paper size in "Setup", it still remains as letter. It's not the printer, I can print 6 x 12" documents in other programs. I've reinstalled both the printer driver and InDesign, with no change. I've reset preferences.

Does your printer actually support a 6x12 paper size? Not all apps can force a nonstandard size to a printer, and there are good reasons you don't want them to.
To turn that around, you have to set up your printer to expect and handle the paper size, whether it's a standard selection or a custom setup (which should then probably be saved as a default preset for convenience). Then you should be able to tell ID (and most other apps) to use that paper size for output.
You may also have more luck exporting to that page size in PDF, and printing from Acrobat; application print settings seem to be drifting further and further from advanced direct control features as PDF becomes the standard print-export medium.

Thanks. My printer absolutely prints 6 x 12" and since it prints from other programs, I'm assuming this is an Adobe issue, not my printer. Thanks for the PDF suggestion. I can do that, but it is annoying that InDesign is "fussy" as Robert at ID-Tasker said. For what we pay for the software, such a simple task should not require a workaround. Sigh.

InDesign is very fussy, so, as @James Gifford—NitroPress already suggested - it's best to export to PDF and then print from Acrobat.
