Data Merge from Excel to InDesign

Hi there,

I need to complete a number of completion of program certificates in InDesign using the data merge.

I have a spreadsheet that contains all the names, and a template (attached). There are five fields that need to be updated: recipient's name (in two locations), coupon expiration date (in two locations), and coupon issue date.  I'm having a hard time figuring out how to do this. 

Any assistance would be appreciated!
