Is there no way to group guides in InDesign? Group is greyed out when I select two or more guides.
(InDesign Version 16.0.1 x64 on Windows 10 Pro Version 20H2)
I don't think it has ever been possible. Why would you need to do this? If you need to lock or hide some guides, you can use different layers.

Even if they are not grouped, you can select more than one guide and move them together, either with the arrow keys or by entering a numeric value in the x or y field in the control panel.
Yes, but I can't use Align and Distribute to reposition them.

I want to be able to create custom columns with gutters by distributing pairs of guides across areas of the page. Does that make sense? (Not that it matters – you've already answered the question: You cannot group guides in InDesign.)

Once your grid is done, select the pair of guides you want to move and indicate their new position, using the x or y field
We also could consider a "group" of guides with an additional page on the spread in a non-facing pages document. The page itself will act as a group for the guides. You can move the page with the guides around using the Page Tool. You even can move the page on top of other pages in the spread.
Now, aligning that selected page is only possible with aligning it to another page. For this you add the other page to your selection using the Shift key when the Page tool is still active. Then you could use the Alignment panel to align both pages:
