You can set the text frame’s first line position via Text Frame Options>Baseline Obtions>First Baseline Offset. If you want the first lines of the columns to align on the baselines set it to Leading

Also, do you want the text to align to the corner shape or do you want them square? You might set up Layers to keep the text and background separate—see attached

Hi Hendy,

Perhaps the first line has a greater leading value?

Are you controlling the paragraphs with a paragraph style that defines the Leading?

In InDesign Preferences > Type, would you like to switch on Apply Leading to Entire Paragraphs?

You use a Hebrew text inside.
Perhaps you use a Character Style. Keep the leading field of that style empty (undefined).

There is a differnce, though, in the two examples. In the split column your first column has both large and small text on the first line, but only small text on the first line of the second column. In the second example both collumns have both large and small text on the first line.

Are you using align to grid?  I suyspect waht you are seeing is the result of using a first baseline setting that is other than a fixed value (i.e. one of the choices related to the phyical size of the type), and the largerr type is pushing the first baseline down in the mixed size columns.

Using an actual baseline grid and a style that forces alignment to it would probably solve this, or pehaps changing the first baseline option to a fixed value in the text frame options.

Can you show us the grid lines? It looks to me like the grid spacing might be half the leading value and and the columns are using alternate lines when one has large text and one does not.

Do you perhaps also have a grid set for the text frame itself?

Can you share that page that shows the problem?

It's hard to see in the screen shot, but it looks to me like there is a different leading applied to some paragraphs.

I like your idea about using align to grid, but why is it aligning to every other line??

OK, I've had a look at the file.

Much of your text to which you've assigned 01 text column paragraph style shows a local override on the leading of 13 pts instead of 12. Some of your other styles have no align to grid option selected. The leading override is probably causing most of your misialignments.

yes, i do want it aligned this way, but not the way you have the columns set up.

this is still the way it looks even if the leading is 12pt, how can I change the leading override in my document?

You can set the text frame’s first line position via Text Frame Options>Baseline Obtions>First Baseline Offset. If you want the first lines of the columns to align on the baselines set it to Leading
