I like to announce an extension and Nodejs server that I have been developing over a decade ago. This software connects Creative Cloud specially InDesign to an online interface. Where the end-user like customers or collaborators can fill and modify dynamic forms. Building templates requires no programming and can be customized without limits. So, InDesign app responds directly to the end-user over the Internet.
You can install Tree Shade to your computer and setup as a server that saves effort and time.

As an example, in this link, you can choose a design layout and enter the content so that my personal computer responds by copying the files to unique path for you and generates the new designs.

If I understand this correctly then the solution connects a user on the internet to connect to a desktop InDesign to create the template based on the form filled? Or is this powered by an InDesign Server instances?

As a designer you create the template in Adobe InDesign and Adobe Bridge. Send the URL link to the customer for instance and then he can update the same document or create new version as you setup the template.

The online form for update the content or creating new instances of the design, and it's not an online design service like Adobe Express. Using InDesign you setup the content as form fields to be used to update the docuemnt or create new copy.
