Weird rectangles appear when creating a square

Something strange happened to your default corner options. Go to Object > Corner Options and make sure that all corners are a simple straight angle:

If this happens in new documents, first close all documents, then set the corner options. And exit and restart INDesign to make these corners options the new default.
