Caption way below image

When I generate a caption for an image from the image, the caption appears several points below the image rather than right below it.  This was not always true.  I must have changed a setting somewhere but so far no luck finding out where.

Anyone run across this?

Object>Captions>Caption Setup. There is an offset setting in the window.
