Headers and page numbers missing or only partially showing up

I have started this book from scratch several times and I keep getting this issue. As you can see, the headers are randomly cut off or missing in some chapters, and page numbers, too. When I first created the review copy, I didn't have this issue. Using the same template - how can I fix it?

You have very thick white paragraph rules (above and below) in your body text, it hides the header:

BTW, you should not have a “white” color in your file. And no need to have 12 different master pages, one is enough. Just use a section marker to change the header.

As I wrote, you have to:

  1. display the paragraph styles panel

  2. select the Body A style, then right click on it

  3. locate the Paragraph rules tab

  4. untick the rule above and rule below checkboxes.

Can  you check your master page? It looks like you have on the master page on the top layer some frame/object.

I checked and there is no frame. This Master was created the same as the others (ten different stories, ten Masters) and two of them are doing this.

Unfortunately a hard problem without seing a file. I would check for paragraph rules that are causing this. It is always harder in a template that you have not made yourself.
Some questions are:
What view mode are you using?

Is there any effects on the frame (on the master page)?

Is the header on the top most layer?

Are you using GPU preview or CPU preview?

View mode - fit spread in window

No effects - just page numbers and headers

No layers - just one master page used

Not sure where to find which preview.

The template is one we created ourselves and have used it for years for all 6x9 books.

You don't look at the right panel. You are showing the Property panel and you have to look at the Paragraph Style panel.

Did you find the paragraph styles panel? Select the Body A style and right click on it.

I have a question (don't take it bad): what is your knowledge of InDesign? Have you ever worked with paragraph styles?

Just the basics to format print books for the past ten years. I'm sorry, but you will have to tell me specifically what to click on in the menu to find that.
