How do I create spread-specific headings like in a dictionary?

I'm typesetting a giant reference book that is essentially a long list of names with short descriptions. I'd like to include the first and last names on each spread in the headings for easy reference, since there are no chapters. Is there a way to automate this? There are almost 2000 entries, plus if my client edits/adds/removes something then it might change all of the pagination, so I really don't want to do it all manually.
