Is there no way to group guides in InDesign? Group is greyed out when I select two or more guides.
(InDesign Version 16.0.1 x64 on Windows 10 Pro Version 20H2)

I don't think it has ever been possible. Why would you need to do this? If you need to lock or hide some guides, you can use different layers.

So that I can maintain consistent spacing between pairs of guides while I adjust their position.

Even if they are not grouped, you can select more than one guide and move them together, either with the arrow keys or by entering a numeric value in the x or y field in the control panel.

Yes, but can you use it for pairs of guides?
I want to be able to create custom columns with gutters by distributing pairs of guides across areas of the page. Does that make sense? (Not that it matters – you've already answered the question: You cannot group guides in InDesign.)

Once your grid is done, select the pair of guides you want to move and indicate their new position, using the x or y field
