Image issue

After using ID form many years I have come upon a most unusual and vexing situation.

Usual procedure:

Sizing an image, then grouping it with a title and applying a text wrap.

When you drag that image into a block of text the title disappears.  This does not happen to all text blocks only some.

In Text Frame Options tick (or untick) I can't remember which (I'm in bed!) Ignore Text Wrap.
