I have an equipment inspeciton form with check boxes for 3 types of condtion (OK, Operational, Critical). I've been asked if there is a way to check one condition and have the others locked-out. For example: Heater - Check Operational / OK and Critical would NOT be able to be checked.

I've attached a PDF of the form for reference.
