Indexing orphans

Firstly, I am very much self-taught with InDesign and only use it at a very basic level. I am having a problem with the index presentation. I want to ensure that the Index Section Head (A,B,C, etc) is not left isolated at the bottom of a page or column. Likewise I would like Level 1 (in my case this is a surname) to keep with Level 2 (given names).

I thought the Keep option would solve these problems. Maybe I am not using this feature correctly?
