Text Anchor Hyperlink Whack-a-Mole

I have a table of contents where each line hyperlinks to a text anchor on individual pages, one-per.

I'm finding that after making my interactive PDFs clicking A won't take me to A ... it'll take me to B, even though the Edit Hyperlink says it should be going to A.

I've gotten to the point where I've deleted and started over and fixed the problems (not every hyperlink goes to the wrong destination) only to find that PREVIOUSLY working links now don't.  I'm just chasing errors now but I refuse to just keep doing this.

Is this a behavior someone is familiar with and it's a simple, “oh, you forgot to check/press/set blah blah blah...”?

The only thing I can think of is to check the Hyperlink Destination Options to see if changing the type from Text Anchor to Page or URL makes a difference.
