I set up a custom keyboard shortcut which shows correctly within the LR Classic menu (see screenshot) and works correctly if you mouse click to the item on the menu, but doesn't work as a keyboard shortcut. It behaves as if the ctrl key isn't working (which it clearly is because I used it to define the keyboard shortcut).
If it isn't obvious from the screenshots, I set ctrl+9 to set the colour label to Purple and ctrl+0 to set the color label to none.
A bug? Or am I doing something wrong?

Instead of "Photo->Set Color Label->Purple" in App Shortcuts, use "Purple". The hierarchical naming of menu commands was introduced into System Settings a couple years ago, I think, and LR isn't compatible with it.

I tried that the first time and I had the same result.
Also, "Purple" applies to more than one shortcut: Photo>Set color label; and Library>Filter by colour label. So its not to clear (to Lightroom) which one the shortcut relates to.

Right, without the hierarchical menu path, the shortcut appears in the menu under both Set Rating > None and Set Color Label None. And typing the shortcut invokes the first one, Set Rating > None.
I tested hierarchical menus with shortcuts thoroughly last year, and I dug up the post:
I observed then that hierarchical menus worked with some menus (including Library > Photo) but not others. So just now, I tried assigning Ctrl 9 to Photo > Set Color Label > Purple:
They work in my LR 13.1 / Mac OS 14.1, hmm. I see you're on Mac OS 12, that could possibly be an important difference.
I suggest some magic incantations:
- Delete the shortcuts from System Settings.
- Restart your computer.
- Re-enter the shortcuts hierarchically, being very careful not to have leading or trailing spaces (System Settings won't remove them).

Thanks @johnrellis, I followed your advice above but no joy I'm afraid. Menu is showing shortcut correctly (as in my first screenshots) but Lightroom is behaving as if the CTRL key has not been pressed. i.e. ^0 sets rating to zero stars; and ^9 sets color label to Blue. Surely this is a LR bug?

I don't know what accounts for the difference between our two situations.
Are you typing 9 on the top row of keys or on the numeric keypad?
Are you on a US English keyboard or a different language keyboard?
In which language are you running Mac OS?
Unfortunately, Adobe likely won't pay any attention to problems with changing keyboard shortcuts.

It's one of the smaller Apple Bluetooth keyboards with no number pad - so just the top row numbers

That doesn't sound suspicious. There have been issues with French AZERTY keyboards and their numeric keypads and with non-English keyboards and accent keys.
