LR to PS to LR Significant Rendering Problem with Edits

Please see the attached files.  I opened the file from LR to Photoshop.  Did a series of edits then saved and went back to LR.  The file in LR appears corrupt or has some sort of issue with the edits and it has corrupted the original and the copy I was working on.  See attached screen shots!  I was able to re open the file in Photoshop and the the issues are not present in photoshop and I was able to save the file as a jpeg and reimport into lightroom with no errors with the copy jpg.  When I opened the file back in lightroom the crop was different from what the file was cropped as and the strange image highlight ghost appears to be from a mask in light room of the clothes from the original uncropped image.  The image when edited in Photoshop was on a crop of the orginal image.

Is your original photo a PSD file? On the image you state is your original, I see two things, one a PSD file, two a stack indicator. Are you actually looking at the original or a PSD copy stacked on top?

Original file was a NEF from a Z9, its not shown in the stack that is in the screen grab.  The photo was first edited as a NEF in LR then opened and edited in PS.  Upon returning to LR the strange artifacts from the orginial mask appeared and the dimensions of the crop went haywire.  It changed the dimensions of the photo crop and "squished" the file vertically.
