Won't import some images even when Don't Import Suspected Duplicates is checked

Importing from Compact Flash via USB reader

Import thumbnails show grayed out images, The Don't Import Suspected Duplicates is NOT checked.
Should import all photos yet does not import the grayed out images

I'm stumped because I've started to encounter some photos not importing that are grayed out as if they are duplicates. This has not been an issue until last week or two.  I do NOT have the box checked that reads "Don't Import Suspected Duplicates" and yet it is still graying out some of my photos.

I'm having to do a complex workaround of copying the RAW .cr2 images that are grayed out to a folder, re-naming them with a B at the end and then it will import them.

With a library of over a quarter million images, I re-use cards frequently and the filenames repeat.  Has never been an issue until now.  And strangely, some images from a shoot will import fine, but a subset of 60-70 images gray out and won't import with the rest.

Anyone else encountered or have a suggestion to avoid?
