Folder structure in library differs from explorer (Windows) after import

Import process:
1. Select source (memory card)
2. Create a copy
3. Goal = create a new folder

Folder display in LRC:

Folder is displayed outside of the normal folder structure in LRC. Folders are created twice.
Explorer:
In explorer folder is shown correctly in normal structure. 

Editing in PS:

If an image was transferred from LRC to PS for the purpose of editing 
and saved again in the normal folder structure after editing, then LRC 
shows this image in a newly created folder.

Who can help? What is wrong?

When you edit the image in Photoshop, do you use the Save command to save the image? You probably know that you don't want to use Save As. Just asking to make sure.

Yes, I am using SAVE. Should I not use SAVE?
