Apologies for the slow response here, I was on holiday last week.
Are you using Apple's camera, or the camera inside Lightroom?
If you are using Apple's camera, can you check iOS Settings > Privacy > Location Services > Camera, and make sure "While using" is enabled? Also, you can see the same setting for Lightroom here too.
Does this help? If not, can you share what iOS version and device model # you are using?

I am using Lr camera and location services are enabled for both iPhone camera and Lightroom.
I am using an iPhone SE 2nd generation. iOS 15.1. Lr 7.0.2. I had the same problem with an older iPhone 6s, same software.
Photos taken with iOS camera are correctly geotagged. The iOS compas coordinates are correct.
Lr has a beaviour difficoult to categorize. It appears to be random, which doesn't make sense.
The coordinates Lr desktop shows are 'very' wrong, sometimes they spot a location a few kilometers far from the real one.
Could it be a problem due to the poor connection to the phone network? Wrong geotagging happens most of the times while I am hiking in areas where the cell phome network is poor. Just guessing.

Your comment here makes me think the camera might be using the location info from where network/GPS is restored for the device. For documentation purposes, I attach here some "worst case scenario" images taken with an iPhone 6s; both iOS and Lr were updated at the latest release availabe at the time.
It is a sequence of 5 images taken in a few second (all metadata included) to be stiched in in pano.
All the images are wrongly geotagged.
Beside that, one in the middle of the sequence is geotagged differently if compared with the others. That's very wierd.
All the images are geotagged approximately in the same area (46°54'37" N 8°28'49" E), which is about 26 ml far from the right one, that is 46°32′42.049″N 8°28′57.562″E . Pls see the attacched Google Maps screenshot.
Even more wierd the GPS and altitude data are not consistent. Since altitude is not available in Lr, if you check the images GPS metadata using PS File Info you'll see that the location altitude in approximately the correct one of the actual location (around 2550 m) and, it goes without saying, it is wrong if referred to the location reconded in the metadata themselves (all the area is below 2000 m).
To confirm, you see the native iOS camera record good GPS data? Our app uses platform APIs, so our camera should have the same behavior as the native iOS camera.
Are you capturing HDR images? If so, do you see this only with HDR captures, or with all kinds of images like regular DNG and JPGs from Auto or Pro mode?
If this is with HDR images only, perhaps this is a matter of processing delay. Lightroom HDR images are processed only after exiting the LR camera and returning to the main view in Lightroom. Is it possible you are capturing HDR images, but staying in the camera and NOT letting the HDRs process immediately?
[Edit] Also, just to confirm, you feel this happens in areas with weaker cellular signal?
The photos were taken in a quick sequence. Metadata included.
- IMG_0054 is the screenshot of the compass, same location as above photos, a few seconds after the sequence was taken. BTW, as you can see, the cellular connection wasn't weak, but no data connection.
