Importing images - duplicates

Trying to import photos that Lightroom reads as a duplicate. Some files haven't been imported (can't be found via file name search and aren't visible anywhere. As a result I simply can't import a number of photos. Very frustrating. Anyone face similar problems? Thx.

Can you provide some more information.... which version of Lightroom and OS etc.?
